Established in 2002 by the WV Legislature, the West Virginia Center for End-of-Life Care provides coordination, education, and resources to help the state with all advance care planning needs. The Center also manages the WV e-Directive Registry which securely stores patients' advance directives (Living Wills, Medical Power of Attorney, Combined LW/MPOA), medical orders (POST form, DNR card), and other documents and makes them readily available to treating healthcare providers so that patients' wishes for medical treatment are recognized and respected regardless of the medical setting.

The WV e-Directive Registry is also the most comprehensive database of its kind in the nation.
